INSTRUCTIONS FOR FILLING OUT FORM 1583
FORM 1583 – DESCRIPTION AND OVERVIEW
The United States Postal Service (USPS) has created rules regarding how packages and correspondence should be delivered and received. All companies who provide mail services as a third party service are bound by these rules.
Emcon is considered a CMRA (Commercial Mail Receiving Agent). All individuals and businesses using our services must complete a 1583 form. This form provides us the legal authorization required to process your mail. The form must be filled out and signed by the customer prior to our company assigning mailboxes and providing services.
WHO NEEDS TO FILL OUT A FORM 1583?
Any individual or business that chooses to have a CMRA handle their mail and package deliveries is required to complete a 1583 form.
This is a USPS requirement and gives us the authorization necessary to provide you with mail processing services.

INSTRUCTIONS ON HOW TO COMPLETE FORM 1583
Form 1583 has 3 sections. The first section requests information about you, the second section is information about the CMRA who will be processing your mail and third section is about your business, if there is one.
Box 1 – Enter the date you are completing the form
Box 2 – Enter the names of the individuals (or the business) that the mailbox will be registered to.
Box 3 – Fill out the mailbox information given to you when you created your account at Mainmailer.com
Box 4 – Do not complete this section. One of our representatives will do complete this information.
Box 5 – Sign your name. This serves as the CMRA authorization.
Box 6 & 7 – Enter your full name and mailing address. This is usually the same asBox 2
Box 8 – Select which forms of ID you will be using as proof of identity along with ID numbers. Please make sure one of the IDs is a photo ID.
Box 9 - This box is only if you are completing form for a business.
Box 10a-10e – Complete the contact information for the business in this box.
Box 11 – Specify what industry your business is in –(ex. technology, fashion etc.)
Box 12 – The names of any person who will be receiving mail at this address should be indicated here. There is no limit to the number of people who can be listed in the section.
SUBMITTING FORM 1583
This form must be notarized unless the form was signed in front of one of our company representatives. The notarization serves to prove your identity. Our company requires that the certification with your signature be attached as a separate document to form 1583.
If you do not know where to find a notary, check your bank. They will typically provide notary services to their customers. Also, search online and you will find a listing of notary publics in your area.
**For those living overseas,embassies and banks usually offer notary services. An official government seal is acceptable if you cannot find a notary. You may also choose to have your attorney type up a quick letter attesting to your identity.
You have two options for submitting the form and documentation.
Regular Mail – Use the address provided under our “Contact Us” page on our website. Do not forget to include all of the support documentation and copies of IDs.
E-mail – Send an e-mail to email@example.com. All documents and photos should be scanned with high-resolution settings to ensure clarity. Attach all items to e-mail.
ACCEPTABLE FORMS OF ID
- Vehicle Registration
- Voter ID
- Government issued ID or driver’s license
- Green card/naturalization ID
- Company ID
- Insurance policy
- Copy of a recent lease, mortgage or deed
Birth certificates and social security cards are not acceptable forms of ID for these purposes.
